To me it sounds like the speaker is singing to someone he is in love with who is struggling with depression or a secret past filled with regret. And he is kind of helplessly saying, "Please talk to me" and "you're not alone" but he knows that she/he can't hear him, and wont communicate, and it is ruining their relationship. Maybe the other party has even suggested that they go their separate ways. And the singer knows that the only thing that will make her/him better is to 'come clean' so-to-speak. To let go of the power of those secrets by revealing them. And he/she is saying that he/she will listen and not judge, and will be there no matter what, and that he/she wont give up.
