i'm feeling the connection between the lyrics and the title, is that these ulcers from his illness, mr. shiver, as mentioned by Tony Clifton, are the 'children shivering in the river,' which i've taken as a potential metaphor which could be akin to mu supposedly sinking into the atlantic. so to 'make it over the mountain' would be for the ulcers to subside, i.e. cease to exist.
