I like twigs interpretation... but here's another.

Keeping in mind that this whole album is written as a means of dealing with the grief of losing his good friend and roommate, the context of the situation that it was written in has to be considered.

The whole song is about the prison of his addiction and the 'woman' of the tale is the drug he's on. The first verse is about the small amount he has, how the drops from the needle look like tears, and how it will be gone by morning. The second verse could be about how the further along the addiction he gets, the less effect the drug has and the 'chimes of a jailors song' the inevitable ringing of his dealers phone for each time he needs a hit.

I think the 'four walled world' is his reality as he's sunken so deep into his addiction that even the simple act of leaving his apartment/room is too much... that he's so addicted that not even the drugs (i.e the girlfriend) can lift him up enough to escape.

The final verse has him trying so hard to feel the elation of the first time, so much so that he overdoses, with tears of despair from not even feeling normal anymore. The slide into night and not seeing anything anymore is him succumbing to the inevitable death by OD.

Maybe instead of Cornell it's his friend who did OD from drugs? It's written from the prospective of the friend who couldn't escape the prison he's made for himself?
