I think it's about keeping secrets and being eaten away by the guilt. Eventually someone finds out about your lies, and they condemn you. The lies might not even be inherently bad, but only told just to get out of hurting someone.

"You've been told you've got a twisted tongue."

The people he/she has hurt with their lies are obviously angry. They accuse this person of being a liar.

"But you can't understand, you're only gonna lose."

For some reason, the liar just keeps on spinning a web of deception in an effort to save him/herself or to avoid confrontation. He/she continues to lie even though it's inevitable that someone will find out in the end.

"Oh, it's slim you snake, your insides made you weep."

The guilt is tearing him/her apart.

"You watched it disappear in tracks She's spittin' mo' blood on your neck You trade your lover for another lover You've been told you are a mother fucker."

The whole ruse blows up in his/her face. The person he/she has been trying to keep things from finally finds out. "Spittin' mo' blood on your neck" could mean a lot of things but I think it's like...yelling at someone, chewing them out before you slam the door and leave. The part about trading lovers could either mean the secret was that he/she was a cheater, or it could mean he/she has moved on to another relationship yet this experience still haunts him/her. "You've been told you are a mother fucker." The last words of the scorned girlfriend/boyfriend echo in this person's mind for a long time.
