IMO, I think the car is a metaphor.. there was no crash or accident, the car is symbolic of how the narrator was "driven" mad by their ex. Why else would he say "it sure felt nice getting ripped apart in that junked out German motor car"? That's taken from the perspective of the victim of the "accident", who obviously wouldn't be around if this were a literal meaning.
Also, as many are probably aware, German cars are known for their luxury. The fact that he describes the car as "junked out" probably means a once great thing has turned sour. To me, this song is about a bad break-up.

Actually, during the period of East/West Germany, East German cars were garbage. Extremely unsafe if you were to get into an accident.
