Face value, it seems to be about strangling someone to death, but there is a distinct lack of hostility in the tone of the chorus. In the whole song, actually, it's more calm than aggressive, so maybe it's a mercy killing? Or maybe just a completely apathetic killer.

Add in the rest of the lines, and I guess it's rough sex, but again, the delivery of the song is calm and strikes me as a little loving.
