Literally it means the blood dripping/spraying from the deer's wound. The shot ("Blattschuss") went through the shoulder blade, therefore the blood trickles on one of the deer's knees, just below the wound.
Metaphorically it very probably means the blood which drips from the girl's/young woman's vagina, because of having sex for the first time. The "Schmalwild" is a young deer and stands for the young woman/girl who most probably was still a virgin. Another possible meaning could be that the blood comes out of the vagina, because the female person is in her menstruation. But this is not quite likely, because it is pointed out that the deer is very young ("Schmalwild")
