The first time I listened to this the first thing that came to mind was "OMG this is about her losing her BFF-ship with Demi Lovato." I read somewhere that Selena did an interview and said it's not necessarily about a romantic relationship, so that only adds to my argument. But anyway:

Selena and Demi met when they were what, 6 or 7, on Barney. I'd say a 6 or 7 year old would most definitely be naive.

Can you imagine how many secrets they'd have shared, how many insecurities they'd told one another about? I think this line is saying that she knows all of these problems Demi is/was struggling with, and she (Selena) still worries about Demi in her heart. Like maybe the reason Demi's still in her heart is because God will have it that Selena will help Demi somehow in the future.
