I disagree. I am an Ozzy fan, have been for roughly 30 years. I am also an alcoholic and drug addict in Recovery. He's talking about the drugs, man. And the high, and then, the downer. If you were in recovery, it'd be obvious to you too. Although I do agree he gets almost the same thrill out of being on the stage, in the spotlight, nothing compares to the mind-altering fuck that addiction brings. The good and the bad.
The line "I know if I don't stop I won't feel guilty" refers to the feeling we get coming down off of a relapse, or rather, the need to stay high so we don't come down and feel that.
