I think it may be about not knowing what happens after you die. He speaks of a few different derivatives, like going to the sky, charon (the gondola man to the underworld).

It also has a really medievel aura to it. In Greek mythology and around those times, you had to put coins on someone who passes away to pay Charon, to get into the underworld. If this did not happen, they would be forbidden entrance into the underworld, I'm not sure where the 100 years of disease comes from.

Or it may just be about dying, simply. This paragraph details people boarding the ship to the underworld, which again, Charon is the captain of, and you must pay him for the ride.
