I think this song talks about somebody who has walked away from God. "It's in my face How much I've changed".
He knows that he needs to get back on the right path, and he's calling on God to help him. "Get me out of this place"
The chorus, I believe, is God replying, telling him not to forget this. He doesn't want him to fall back into his previous life. He is saying to write it down, to scream it out, whatever necessary to remember.
In the last verse, he is saying that he was broken, nothing left, yet God showed him love and helped him through.
So, in short, I see this as a continuous struggle that is happening with many people. We just need to remember to keep on praying and working at it, and God will help us.
