She doesn't have a disease or sickness (definitely not IBS), it's not about drugs, and it's not about sex.
She clearly puts up a front and keeps her true feelings inside of her. She acts like she is in control by putting a smile on. She gives her friends and family the idea that she is ok and she knows what she's doing with her life (control) and has no worries, but this is all false.
The beautiful thing about music is that it can not only bring people together but at the same time everyone can have their interpretation. It isn't something to argue about. Music let's you reflect the feeling it gives you.

Personally for me, I can easily identify this song with my Bipolar. And that isn't because it was played in a movie trailer about a mental institute. It was a good book though. But I can also see how it can relate to drugs, sex, gay rights, and simply just wearing a mask when you feel you're in a world chaos....

There's no real right or wrong answer when it comes down to what YOU feel.
