As far as a meaning, I think it's fairly straightforward: The dude's ready to go home. It sounds like he's had a helluva time in L.A., and now he's had enough of that lifestyle. He can't leave on his own, for many possible reasons. But he knows that his lifestyle's unhealthy, so he HAS to leave.
