To me this song is about your inner/personal "demons", this of course varies from person to person but basically any tough situations you are going through that keeps pulling you down (fear, anxiety, depression, traumatic events, addictions, stress, guilt, etc)
It sounds like the kind of enemies one has to overcome to keep moving forward.

He developed Wernicke–Korsakoff syndrome as a result of it, and Dear Agony is the first album after he went through sobering up and recovery. I believe this is an inner dialogue with his alcoholism still trying to tear him down. The first lyrics are about him searching for a hidden bottle and being ready to drink it all it seems.
