I think this song is about a girl who is in an abusive relationship that has a tragic end. On the surface it does sound like a song about a girl who is just heartbroken, but I think most Delerium songs have a deeper, more thought provoking meaning.
She knows the voices in her mind
They tell her
To leave
(she is in a bad situation and she knows she should leave)
She’s tired of smiling madly
Until silence becomes very silently
A noise in her mind
(Faking the happiness to every1 around her. She has kept quiet about her situation for so long, but it's constantly on her mind)
After all she has nothing inside
No good to give
No meaning to live
The mist engulfed tonight
Every single star possibly her death. (How she feels about herself after taking the abuse for so long, she's lost the will to live. I think this is a foresight into an explosive night, Stars representing the spark of life and being smothered or unable to see)
After all they shared
How could he simply say no

He turned away from her to go
She said she wouldn’t cry
That was really just a lie
(He is saying no to her life. She feels like since she has endured his abuse and he has "put up" with her they have shared a lot. She said she wouldn't cry because that makes him angry or he told her not to, but she did cry. So since she "lied" to him, he turned away from her in an abusive, angry way.
She knows the noises in her mind
Nothing’s left but
Screaming silence
(She hears herself screaming, nothing is left of her inside her mind but the screaming)
After all she feels numbness inside
The feeling’s gone
She’s upside down
The pictures behind her closed eyes
This time you went too far
(Her body and mind are numb to the abuse and fear. She's dying and it all replays in her mind even as she's dying. He went to far with it this time and kills her.)

@tambellina I dont agree that she is in abusive relationship, he is leaving her, so maybe is about another thing.
