"Tell them all I know now" - tell everyone how you cheated/never loved me, tell everyone how I fell for you, then go humiliated. Go ahead, tell EVERYONE. It can't make it any worse.
"All my scars are open" - maybe she got hurt before?
"Tell them I was happy, and my heart is broken" - She used to be really happy with him, but then he went and broke her heart
"Tell them what I hoped would be impossible" - maybe she had hopes of a future for them (marriage, kids, etc) but after the breakup, it became obvious that he never had that intention, so it was "impossible" that that would ever happen
(I disagree with the lyrics: "when all is gone" should be "all is done")
