Whether the correct lyrics are "now" or "not", I defiantly hear "starving." As this song explains some sort of eating disorder, it would only make sense.
I think this song is about a girl that pressures herself to be "perfect" and indulges herself in vanity... she's obsessive to please him. The girl he's describing in the song seems to be bulimic and he only blames it on himself that she is this way.

If the lyrics I'm guessing are correct, I would say that this displays some sort of affection in trying to be the bigger person in the situation. Being in his position, something like this would make me feel the same exact way... you'd feel like you wouldn't want to end up like the person who's sick and you'd feel bad for that person. The only thing you could do is stare away and try to pretend everything is okay. And obviously, you wouldn't want to eat in front of someone with an eating disorder, so that's why he's "faking it."
I see this line as a conversation between them after she put back on her clothes...
