I think some of the options people have posted are all vialble interpretations of the song, no doubt. That is the beauty of music, that a song can mean something different to each individual and that individual can relate it to their own life in one way or another. To me, this song is about a person in a relationship with someone she truly, truly cares about, deeply. However, she's screwed up...made some mistakes, I believe she is referring to her breaking the trust in the relationship. She knows she should confess and tell the other person everything she has done, but she also knows that this will deeply hurt that person and that the relationship will be over.
