To me it feels like this person who's singing actually SAW how cruel that one can be. That he/she SEES him/her true appearance.
I'm not sure how to explain it, but... try to imagine that these two in the lyrics actually know each other, that they even were in some kind of relationship. Suddenly one of them is hurt by the other one. He's afraid of talking to the one who hurt him, he won't be able to open to that one person anymore, or to trust him. And though he dreams they could be together once again, though this person means a whole world to him, he doesn't want to take chances anymore, in fear of risking his way of seeing this person. He doesn't want to conceive this person as cruel, doesn't want to admit it, but he perceives it anyway
