I'm pretty sure this song is about taking antidepressants or some other mood stablizer (Patrick recently told a crowd at their show in San Francisco that he takes Lexapro; http://www.spinner.com/2010/03/29/titus-andronicus/) and he gave up what he thought was true (or being his true self) and now he's like a... well, a robot, but he still hears that voice that pills just can't silence.
i agree - the lou reed/vu waiting for my man reference in the pharmacy is a pretty clear indicator - and the man down below (in the dungeon) doesn't respond to anti-depressants and still says "you will always be a loser :)" - i can identify with this sentiment - always wanting to be better than you are - thinking you suck (even if you don't) could be a good strategy to make yourself better - or just more depressed.
