To me, this is a song about unconditional, unrequited love. A man is so devotedly and helplessly in love with someone that he will continue to love them far after they've gone and forgotten about him--even though it tears him apart. I get the feeling that this person is no longer in his life and it's killing him to love her but he does it anyway. Sounds like he can't help it and even wants it this way.
