Captures the feeling of being lost and annoyed with yourself for not being able to do the things to help yourself be happy. It's a frantic feeling, searching for something, when you know (or don't know) that it's really within yourself to be content. "I've lost my mind, but the world is wide" the world is still immensely vast and time is infinite, but you're all tripped up inside your own head. And then you feel like the last verse of the song and you give up.

As a sufferer, for me this song could certainly be about a depersonalization or dissociative disorder, something that is typical a symptom of highly level anxiety and even depression. Given that he comments on a doctor, that all his vital signs are in check, yet he still feels strange to himself and lost in the universe. "Something's coming over me, like I've lost my mind, but the world is open wide" is an eerily accurate description of how DP feels.
