It does sound like alcoholism. The alcoholic is upset with Ian's efforts to stop them from doing what they love ("I know you got roses, You want to throw on my grave"). He doesn't want them to hurt anymore ("I'd rather see you hit me"), but he's out of ideas ("I was wasted at the fork roads, I'm too old to run"), and all of the alcoholics reasons are weak for quitting ("Words and excuses lame"). All Ian feels he can do is "Leave" and/or "Bleed" for his friend.
