I read somewhere (and this was when it came out in '93 so my memory may not be perfect) that Scott Ian said it was about a mother didn't abort her illegitamite son because of her religion. He then grew up with a mother who was too young to parent and became a serial killer or something. The song is the killer on death row telling his mom she should have aborted him.
