This is one of my favorite songs in their album LaoD. But anyways this is what I think the song means
Song Title: Lets look at the song title, Affliction. It means something causing pain, or agony.
Beginning: That the singer is lonely and doesnt fit in with anyone, and no one really wants him.
Middle: That there are people that are causing pain to him and that everyone thinks its alright to do it, since hes an outcast.
Ending: That in the long run the people that are doing this to him will get payback, or "Pay what they owe".
Thats what I think the song means, im most likely wrong tho >.<
