I think that he'd "tear out his eyes" for her not because he's madly infatuated with her, but because he wants to give this cold-hearted woman a sense of sight so that she can see all of the pain she brings upon others.
Not to say that he never was infatuated with her. I think that this was the case in the beginning, then she tore his heart out, and now he wants her to see and understand what she's done.
Lastly, though knowing that "nothing's lost", when she leaves him, he knows that he will still grieve because of all the pain he feels -- while she's able to leave KNOWING that nothing is lost because it all meant nothing to her, and soon doing the same thing to someone else.
