This is just my interpretation though, and i believe it may be right. I don't think mark arm wrote it about himself, because i've never heard about him having a heroin problem. But, being part of the whole seattle music thing, a lot of his close friends did. (he was friends with people like kurt cobain, layne staley ect..) So he could have wrote it about one of his friends that were in a disfunctional relationship with a significant other.
