I don't think Mary is a physical person in the song tbh.
I think she's either a split personality ("Mary, I was looking in the mirror for you"), or a memory he's obsessed with (presumably an ex lover.)
I think either that side of the personality, or his recreation of that ex, twisted him to do something wrong (either kill her, or do some crime for her) and he's suffering for it, either in prison, or in some other way. Maybe the religious connotations of the song are a reference to people who think they've seen figures of, or been possessed by the virgin Mary?
