I think this is the end. First Caleb, then "The end" were he finaly finds her back, then "Don't say" where he express his anger and tells how he's going to kill her. In Juliet, it seems that Caleb didn't kill her in Don't say a word, not yet. I'd say that Caleb is accusing her of "killing him" because when she left him, he felt like he had no reason to live on, while her was going away, smiling.

To me, those lines means that he expecteed her to "die" with him when they separated, to suffer like him.
