I think the first two lines are symbolizing the way we see things that aren't there. A hard-to-reach malaria is saying how its very hard to relate to since so few people have Schizophrenia. Since most people don't know anything about Schizophrenia other then the fact they have voices in their heads, it's scary to them since they know nothing about it.

People assume to have a mental illness such as Schizophrenia that you'll have to have a long story about how you were mistreated your whole life and went through so many traumatizing experiences, but this isn't necessarily true. Sometimes it's totally unexpected. You could grow up being totally normal and happy, having a completely different personality (strong and desperately brave)but all of a sudden Schizophrenia takes over.
