I always assumed the "Tilt back your head..." line was a reference to both the rape and the overdose: he's forcing himself down her throat/her to swallow with the same intensity that he forces himself to swallow his poison-of-choice.
It occurs to me now that the rape/murder is probably a fantasy, while the overdose isn't, thus his anguish, "Oh, shit! Fuck!"
(WanganGC8's "O.S.F." speculations seem completely viable too...)
Also, the title doesn't necessarily have anything to do with the lyrics and is quite possibly random and conjured up at the last second.
