The way I see this song, it all comes down to the first line: "Are you in, or are you out?" - It's a question. I think the entire song is basically about a crossroad in life, and he's asking the other person "Are we going to be together or not?"
In a sense I think it's about loneliness, because he's spent his whole life in "The quiet cold of inner darkness" and it's only through the person the song describes, that he's found happiness or come to know himself... or whatever.
And so, even if the other person decides to move on... he will always wait for them, because that's the only place he could ever find himself.
