The line "this ones of me throwing up for you" could be referring to why he was bulimic, ( I read this book once and it said when you have an eating disorder it usually means something else is going on in your life). So, in my opinion, I think this song is about how Josh had a friend or family member who had to go through cancer and he dealt with it through his eating habits.
