i think this part is about this girls bf watching her and noticing that she is with a guy friend and he wants to end that because they look like they are getting too close together. thus "until we wind up dead" and the "checking your pulse" means to make sure she stays away from the guy friend and make sure she is not lying about seeing him.

"till one remaining cold calculated thought" so she stays away from the guy friend and tells him he has to stay away.

so then this is where all of this happens

he is talking about how great this girl is. how she is so much better than the guy she is with hense the part to shy to know your worth she keeps getting hurt by this guy hense the "hurt from the insensitive behavior she is scared of how she feels about this guy or guy friend and how her feelings are toward him and where "this might lead us"

the dont let me go part is about her letting him go obviuosly and him opening up to her and she wont accept it

the it grabbed us by the throat means now that they are opening up the guy cuts them off and they continued to see each other but she wont leave him. the "its not worth it part" is about him giving up and waiting for her and doesnt want to get hurt by him

he is talking about the guy now (i destroy you) he is better than the guy and destroyed him (even through the only things that i have left for your condescending eyes looking at each other) he doesnt have much to offer the girl but the truth and love so he is better and he has opened up to her fully now with the shadows funning from sunlight meaning that he is pointing out everything and doesnt have much time but still wants her badly

so she is breathless and she denies that they are something and if she just wakes up from her nightmare with this guy she would notice this great guy right beside her
