i definitely think this is about his bulimia. it could be about a person but i don't think so, i have bulimia and it sounds he is constantly relapsing. it's like a letter to himself. that he will try harder to recover this time.

i agree. the line, "i'm sorry that i'm always the one to make you feel that burn" definitely seems to be to that effect.
