Basically he is afraid she is going to take her own life because of the abortion she had and he's not gonna let it happen. Though he is definitely not happy with the fact that she did it either.

Hence "I swore not to come but I'm here after all" This also explains the "I feel sorry for you/I'm here for you"---"This is because of you" switches throughout the song, she got the abortion but the doctor feels obliged to help her.
