I'm not sure about the meaning, but in my interpretation, the first part is about someone that doesn't feel affection and is tormented by that, and tormented by not understanding how is that possible. The second part might be two things: 1) the person speaking is the same and is someone that loved "him" but is dragged down by his darkness. 2) is the tormented one speaking.
In both cases, "him", the one with no affection, old, grey, bitter, anxious and collapsed, is the black light district.
