It could be someone who left them "broken hearted" or whatever, again, like a stereotypical emo kid, to do self destructive things because they think it's the "in" way to solve problems (not that other people don't do it). --Later on, I think "she" could also be reffering to the crack and the effects it takes on one's brain. It could be reffering to both at once, but I think it starts out not reffering to the crack. "Nothing hurts anymore" = the euphoric numbness from the crack. "Never mind, then she killed my..." They could put on a new persona any time they felt like it. "I THINK" shows that it's a decision and that it's been contemplated; "should I REALLY do this today?" "If I foret to take my medicine then I'll be sick."-- when people are addicted, their bodies build a resistance to the euphoric effects and it becomes something they need to take just to get by.
