The more discreet meaning, number 2, is that he took heroine again for some reason but only once or something and this is his guilt of taking it or how many bad memories it induced (see: One More of Me where it reverts back to Smiles/Niandra Lades esque music structure and lyrics)
As far as the line itself, "Hit me where I turn white" Is probably where he used to shoot up all the time/the one time he may have gone back to it for whatever reason.
