I'm surprised nobody has posted or commented on this song. Awesome band..
To me, this song is about a guy talking about a girl he recently broke up with. This girl either struggles with some kind of addiction or deep depression, and even though she would reach out for him so much when they were together, it must have been so bad that he felt there was nothing he could do to help her at the time. But now, he is looking back with regret and almost feeling like he could have done more to help her.

And now the memories are slowy fading"
^This is him further showing his deep regret of what he didn't do. He feels like he is the one who needs to be saved now because of how badly it eats him alive, or maybe he feels like he wishes someone would have been like "don't give up dude.. keep trying to help her" while it was happening.
"Wish I could see me through the eyes of someone else."
^Again, if he was another person looking at him and the situation at the time, he feels like he would be able to realize and recognize that he didn't help enough.
"I know I shouldn’t blame myself
For what I cannot control
It seems so impossible to ever let this go"
^deep down, he knows that there probably was nothing he could do to truly help in the end, but there is still that little voice of shame and guilt in his head telling him otherwise.
Just my take.. could be totally wrong. Overall, EXCELLENT track and a great band.
