Honestly, I think it is about God. & I disagree with you PorcelainXombie. "Like the knife you stuck into my back" doesn't have to be a symbol of betrayal in this song. It could mean that God did something in his life to punish him, hurt him and show him he needs to turn back to him and "pick up the pieces".
