It nearly made me cry the first time i heard it, & so i probably automatically think about the things that i've dealt with, especially as my name's Kate anyway.
but i always thought of it as a girl who had an abusive childhood (physically or mentally) 'she was never okay, never found the way.
but always had hope, and said she would be okay, even though she wasn't.
when they said 'the little bit of something, & contorl over me; i always saw it as self harm. which could be anything from cutting to eating disorders to drug/alchohol abuse.
