This is a truly beautiful song, I've listend to it a thousand times and I've never once got sick of it.
Yeah, I assumed that this song is about death as well.

It's not like her heart is getting tired of feeling, but it's like her heart it slowly deteriorating because the cancer/illness/disese is making her weak. The part where is says 'the night has fallen and it's stealing the last of our little life this time' it's like... because it's night time, she'll be asleep and he wants her to be awake, because they don't have much time left to spend together.

It's like, she was sleeping in his arms at night time, then by the time the sun has risen, she has passed away whilst she was asleep.
