I think the "downstream and out of town" part is her leaving all things human...leaving or dying into what is real or natural. he is in the city, clearly gone from her, and is drifting away, and "And I don't know where the river takes you I remember Hollywood" means he's getting caught up in it all..doesnt know where she went..shes a part of his past...losing the memory and getting caught up in stupid society..lost out on something simple and wonderful. Pretty song, one of the best.
