I had thought it was about people with medical conditions that go to get heavy surgery(i.e. Lobotomy) and they end up to get just as fucked up and how medicine and hospitals are fucked up and can make people far worse. But the final line "getting your way with a five pound note" seems to show what others have thought as correct. I never really understood that last line... used to think he was saying "hit on the head with a five pound brick" haha
