It's not suicide or old age he's talking about at all, but something that actually happened. That's probably why this song has such a real feeling to it, the emotions here are genuine. Other songs on related topics are usually a bit weak because the person writing the music and lyrics normally hasn't actually experienced what he's writing about.
Also, I think the music itself conveys the emotions far better than the lyrics alone ever could.
