When I first heard this song, I took the lyrics really literally, so it was just almost describing a dream with this clown in it, (well, probably a nightmare). I love it anyway, it's nice and scary (:
After reading waht other people have said though i can see how it could be about drug use, or child abuse, and it could alos be interpretted as being manic depression, almost...feeling really high one moment as if nothing can touch you, but whats going to happen when you fall back down?
