I'm not sure whether he is pointing the gun at someone and that's why their hands are up (maybe at the "corner store"), or if its an attempt at suicide and someone has their hands up as an instinctive calming response trying to talk him down.
He seems to think that "they" have brainwashed him and the person he is talking to, but presumably everyone is under "their" control (society/media).
