I think the meanings pretty obvious. It's about a man who's not taking proper care of himself.
"He thought he was twice.
He was moving away
cause everyone thinks that it goes away with age."
This part makes me think he may have a mental illness that everyone takes for a phase or no one is listening about. And he's just getting worse.

You keep me safe.
Don't ever think you're the only one
when times are tough in your new age."
Says to me that he's only holding on because he's realized to live for something, or someone. He wants them to hold him and he's safe and he knows it will be alright if he just keeps holding on.
