It's about people having trouble in their relationship- being on and off the whole time. The guy is saying that he really cares about her, but that she needs to decide what she wants from their relationship once and for all because all the indecision is hurting him and if she isn't going to stick at it, he could be moving on and starting to try and get over her. If she is only going to choose not to stay with him, he would rather that she didn't lead him on and make him think that she cares when she doesn't really....because it's only hurting him and making everything worse. It's really sad :(
